This is a friendly reminder that the Tzu Chi Mobile Clinic will be on campus starting Friday, November 14, offering free medical services for students.

Please remember to return the completed forms to the office, your student’s teacher, or Room 2. Extra forms are available in the office if needed. Be sure to fill out and sign both sides of the form.
